Determine your eligibility (see below) based on income and insurance status.
Step 2
Proof of Income
Send proof of income and a picture ID to frontdesk@ncdo.net.
Step 3
Wait for Approval
Wait 5-10 business days for approval.
Step 4
Schedule Your Exam
Schedule your new patient exam. This includes x-rays and a treatment plan created by our dentist.
Step 5
Begin Your Treatment
Begin your treatment. This may include fillings, extractions, root canals, a cleaning, or other procedures on your treatment plan.
Eligibility
Low-to-no Income
All patients must meet our income eligibility (see income verification) to be established as a patient. We use the ALICE threshold for income and a sliding scale for payment.
Dentally Underinsured
We accept patients with Medicaid or no dental insurance. We encourage all patients with other insurance coverage to reach out to our partner dental offices for an appointment.
Income Verification
All established patients must provide a proof of income, including: a copy of their most recent paystub, a statement of benefits (SNAP eligibility, social security statement, etc.), or a signed letter from a case manager or employer.

To become a patient with us, you must provide income verification documents to our staff and allow 5-10 business days for approval. We must see all income documents for everyone living and working in your Household. Example documents for submission are tax returns, a letter from your employer, or a SNAP approval letter. We cannot accept bank statements, Zelle transactions, Cash App transactions, or similar as proof of income.

Our Services and Fees
We aim to serve our community by providing access to basic dental care.
Patients with Medicaid will have their insurance billed.
Patients paying out-of-pocket must pay the nominal fee for their treatment when making an appointment. We do not offer payment plans. Please see the fee schedule based on your income.

Next Steps
Have a dental emergency and need an extraction? We see the first 6 patients on Tuesday nights at 5:00pm and on Wednesday beginning 9:00am (first come, first served). Please arrive early to secure your spot in line.
